It’s not like these guys have to raise their arm into the air before stabbing someone combined with the general distractions of a music festival and a large crowd.

I imagine it’d be a lot like pickpocketing in that regard

and yet there's been decades and decades without an INCIDENT WHERE ONE HUNDRED PLUS people have been pricked with syringes at any of these festivals.

Everyone at these festivals are not drunk/high/distracted by the music so much to the point of not knowing "ow, wtf just pricked me in my arm"

There are designated safe stations at these festivals and people that are meant to look out for their friends in their friend circles.
