My favourite rapper is Nicki (don't at me about this I don't really care about your irrelevant opinion):

Let's all take a moment to reflect:

The bolded line is clearly a quadruple entendre as it references 4 different topics: Dominican Republic (DR), medical doctors (DR) and emergency rooms (ER), how popular (hot) her music is and how “feverish” (sexy, hot) she is.

The Dominican Republic (DR) has a climate that mainly varies between 70° and 90° Fahrenheit, which is pretty hot when you consider that it’s that warm all year (including winter).

The vacationing spot is known for having intense heat waves and a lack of healthcare. In a tropical region when you’re on an island, you’re prone to get ill as a resident and end up in a hospital at a much greater risk when the temperature is very high. This fact is, again, relevant to the line's reference to Emergency Rooms (ERs) and doctors (DRs).

She’s also referring to the show E.R. It is a classic medical drama show involving surgeons and practical doctors. The series was renowned for having dangerous, real-life scenarios.

Also, the show was very well known for having an attractive (hot) cast (actors) of doctors (DR). This is in line with the latter part where she mentions "motherfuckers actin'".

She's also saying that her artistry won’t “ice age” anytime soon and will stay hot as she keeps pumping out hit singles in clubs and radios.

Nicki is also saying that she delivers, not only musically, but in the birth room too, as she can birth or abort the careers of many upcoming (fetal) rappers. This is in line with the latter part where she mentions pushing out babies/rappers and giving them to the clinic, as in she doesn't claim them, she's too embarrassed of them because they're nowhere near her level.

While people say the E.R. doctors are steamy, she says she looks better than them and the DR all by herself.

The E.R./DR references, therefore are “conjoined wordplay”. Her point is that her rapping is so sick that she needs to be rushed to the emergency room (ER) for immediate treatment.

The clear usage of acronym spelling is also a reference to her verse on DJ Khaled's Take It To The Head and it gives way to her making sure that we understand the lines can be interpreted in multiple ways, so the line "When it comes to rap skills, nobody wanna see her/Seer/CR" also explains that if you motherfuckers are seeking some actual rap skills, you sure as hell don't want a Czech Republic/Costa Rica b**** in your ears, you want someone from Trinidad (Nicki is from Trinidad). The acronym being used for a country is not a reach here as she used one for the Dominican Republic in the earlier line.
