• all participation will be collected on a spreadsheet to be presented as raw data at the end of this

• You can submit this form and then come back and edit it later. Take your time to think it over and come back to it later. I've given you all a lot of time. You do NOT have to fill this entire form out and you especially do not have to do it in one sitting
