Gojo up to that point was in control of the fight but he kept losing DE and noticed even though he had the advantage with his OP CT, Sukuna just kept up. He only probably started having those feelings that Sukuna was stronger when the brain rot, and when Maharaga sliced him the first time, because Sukuna wasn’t even using his own techniques but had Gojo killing himself and on a time limit

Sukuna won the first part of the fight, and was ready to kill Gojo, but suffered from unlimited void, which he was fine being hit by. Once you realize Gojo had to escape death like 3 times this fight, you realize how he was actually losing up until the end
