also, the submarine operator story is interesting but kind of irrelevant. nukes were almost launched, but it wasn't because MAD had fallen thru, it was because a technological failure was telling soviets that Americans had launched nukes.
point being, MAD has always held true and will likely continue to hold true. I'm mot saying MAD is foolproof, but I am saying it's there and it means that there is a non-zero chance that war between nuclear powers does not end in nuclear war.
