They very explicitly say that the surgey is not guranteed to create a successful vaccine, but it would almost certainly kill ellie. And Joel's selfish for not wanting to take that chance? After just having lost his daughter?? The beauty of the first game's ending was the ambiguity of it, its open to interpretation of whether Joel was "right or wrong". But play this game and it definitely wants to make that choice for you, which is fine, but it also makes the ending of the first game completely weightless now. You're really gonna sit here and tell me you played that entire first game and felt like the villain afterwards?
