Either could be that Tony will live the rest of his life being paranoid at every waking moment, he suddenly died of a heart attack, or got whacked right there.

Knowing that the last episode is about to end you feel tense and anxious while watching, even during a mundane scene with the Soprano family at a diner. The camera switches between Tony’s perspective and 3rd person several times. I interpreted it as after everything he’s been thru we’re getting a first hand feeling for Tony’s regular state of mind. Even a normal dinner with the family is extremely tense, he can never let his guard down or relax, he’s basically cursed forever to be on his toes for every waking moment.
A lot of people interpreted it as Tony getting killed tho. The camera switches to Tony’s perspective when someone walks into the diner. Right when Meadow walks in, the camera goes black. A lot of people interpreted it as Tony got shot in the head right at that moment, so his perspective is blackness. There’s several references in the show to getting whacked being something that you’ll never see or hear coming, it’s just over. There’s a sketchy looking figure in a members-only jacket that walks in right before AJ. The popular theory is he was a hit man sent by the NY family.

As for one interpretation a friend and I had was that because of his business and what he does, he does get killed but so does AJ and the mom since they're sitting with him.
They play the song by Journey and one part "Just a small town girl
Livin' in a lonely world" referring to the sister since she wasn't in there yet and would survive.
But I always definitely saw it as him getting killed, possibly AJ and the mom too.

It’s left up to interpretation but David Chase deliberately put clues in for the ending.
All the signs point towards Tony getting shot in that last scene but The Sopranos never deals in absolutes so it’s really up to you. It’s all there though especially in season 6.
Even if Tony survived he was going to prison imo because Carlo flipped and was going to testify against him. Bleak ending either way.

Chase is just a genius and rather than showing Tony's brain splattered on the table, he cut to black and forced us to discuss this a decade later.
The first episode of season 6 is called "Members Only" and features Eugene Pontecorvo asking the family if he can get away from the mob life. Long story short, they reject his offer and he ends up hanging himself. The last scene features a man in a Members Only who walks to the bathroom, similar to the famous Godfather scene which AJ has said was Tony's favorite scene. Infact, the two look extremely similar like they could be brothers.
Couple episodes prior Bobby and Tony are discussing death and he says "you don't even see it coming"
When Chrissy woke up from the coma, he said the devil told him 3 o'clock, the man would be coming out of the bathroom from Tony's right side
There's a bunch more of these and David Chase said he knew the ending of the show long before he even shot it. Everything we need to decode is shown in the final scene he was quoted saying.

I’m a huge sopranos fan but there is no better ending than the shield.. the last four episodes of that show are the best television I’ve ever seen in my life
