It's more so about what we already know about diseases and biochemistry. If you want an example of how it works just look at Earth. Not all diseases effect the same species even though we all evolved on the same planet. Now imagine how unlikely it is that a lifeform from another planet is compatible with any terrestrial diseases

I'm thinking the problem will be their DNA, because unless they evolved in an environment that's almost exactly the same as ours then their DNA (or whatever they use to pass genetic information) is going to be too different from ours
