I'll take crowdsourced reviews for food or travel but not medicine. For that I need to see peer( i.e fellow doctors) approval. If you could create this type of system you could create a more distributed health system. You can't abolish the central location though, there must be a birthing area a hospice area an ER etc and A these cannot be family run things and B they require a lot of power and resources.
Logistically, hospitals have to stay, but it may be worth looking at more specialized places i.e burn centers or places specializing in treating impact wounds if for no other reason than to make hospitals less crowded
