When you mean regulation do you speak of the safety measure or more so the insurance aspect of the process of approval for such work?
both tbh including degrees. none of that should be required to practice medicine.
if two people have a mutual agreement govt shouldnt stop it

Assuming the patient gives verbal and written consent to be operated on then whats the issue?
If you go to a surgeon with low reviews thats on you, same way if u go to a restaurant w a s***ty yelp score its on you
